Access http://aklan.gov.ph and fill out the Online Health Declaration form, choose Boracay. 2. Send the following to [email protected] with email subject OHDC- Family Name, First Name(Health Declaration form, RT-PCR Negative Result(taken within 48 hours prior travel date), confirmed booking slip and proof of identification to [email protected]. 3.
