I have stayed for 4 nights at Mallika in January. It is a big room above the garage, so you have no neighbours. Very very soft mat - beware if you have back problems! 
There is all you need- some plates, cups, many chairs. Good that there is mosquite net, mosquiotos are thirsty. 
You can buy drinking water directly from the host- no need to go out for it. Some 15 minutes walk to the Turtle beach. Good place for a weekend.

Spacious room in a quiet area

Room was great, exactly as advertised. Very clean and had mosquito nets on the beds. The bathroom was a little tired-looking but was not an issue. Location was great - away from the hustle and bustle but very close to the Fort and the heart of the city. Recommended for trips with family or friends.

Clean room in a great location

We spent our first two nights in Sri Lanka at Mallika Guesthouse. As arranged previously we were picked up from the airport and brought to Hikkaduwa for 80$. There the owner immediatly wanted us to give him the full ammount of money for our stay- at this moment I still thought this was maybe normal in Sri Lanka... Our room was basic but okay. Just the German-style carpets in the bathroom were quite disgusting. There was a menu for food and drinks with ridiculous prices: a cup of coffee or tea for 4,  breakfast for 8$... 
When we left the house to go to the beach, the owner seemed to have waited for us to offer us his services as a Tuk Tuk driver to some places worth seeing in the surroundings of Hikkaduwa. For a journey of more or less 30km and 3-4 hours he gave us a price of 35$. We promised to think about the offer and tell him later in the evening. However, in the following days every time we came out of our room, he was somewhere around just waiting for us and asked what time we wanted to go on his sightseeing tour. This was really unpleasant and annoying.
